COMPOSITION

The lyophilisate (lyophilized substance from white to yellowish color) contains the active ingredient ademethionine 1,4-butanedisulfonate – 95.0 mg (which corresponds to 50 mg of the ademethionine cation).

Solvent (clear liquid from colorless to pale yellow color) contains L-lysine, sodium hydroxide, water for injections.

Gepax 100 - contains 190 mg of the active substance ademethionine 1,4-butanedisulfonate.

Gepax 200 - contains 380 mg of active substance ademethionine 1,4-butanedisulfonate.

PHARMACOLOGICAL PROPERTIES

Ademethionine (s-adenosyl-l-methionine) is a naturally occurring amino acid that is present in almost all tissues and body fluids. It mainly acts as a coenzyme and methyl group donor in transmethylation reactions, which is a necessary metabolic process in animals.

Transfer of methyl groups (transmethylation) is also a necessary metabolic process in the structure of the double phospholipid layer in cell membranes and promotes membrane fluidity. Ademetionine is able to penetrate the blood-brain barrier.

The process of transmethylation involving ademetionine is key in the formation of neurotransmitters of the central nervous system, including catecholamines (dopamine, norepinephrine, adrenaline), serotonin, melatonin, histamine.

Ademethionine is also a precursor for the formation of physiological sulfurized compounds (cysteine, taurine, glutathione, coenzyme A, etc.) in transsulfuration reactions. Glutathione plays an important role in hepatic detoxification.

Ademethionine increases hepatic glutathione levels in patients with liver damage. Folic acid (folates) and vitamin B12 are essential co-nutrients in the processes of metabolism and regeneration of ademethionine.

After intravenous administration, the pharmacokinetic profile of ademethionine consists of a phase of rapid distribution in tissues and a final elimination phase with a half-life of approximately 1.5 hours. Absorption with intramuscular administration is almost complete (96%), and the maximum concentration in plasma is reached approximately 45 minutes after administration.

INDICATIONS

For dogs and cats, it is used for:

  • intrahepatic cholestasis, including in animals with hepatitis of various etiology in a chronic stage;
  • intrahepatic cholestasis in pregnant animals (except cats);
  • liver disease at all stages of the course of the disease, including cirrhosis (fibrosis) of the liver;
  • intoxication with hepatotoxic substances to enhance metabolism;
  • treatment of enteritis and hepatitis of various genesis in the acute course of the disease as an antioxidant;
  • psychomatic and psychological stress on the nervous system (stress, epilepsy, etc.) as a stabilizing and supportive agent.

ROUTE OF ADMINISTRATION AND DOSAGE

The drug is used intravenously, intramuscularly, or subcutaneously, once a day at a dose of 0.2-0.4 ml (corresponding to 10-20 mg of ademetionine cation) per 1 kg of animal body weight.

The duration of parenteral therapy is 5-10 days.

Before use, the lyophilisate must be dissolved in the supplied solvent immediately before use. This solution should be clear without visible particles, colorless to yellow.

For intravenous administration, the required dose of ademetionine dissolved in a solvent should be diluted in 200 ml of 0.9% sodium chloride solution or 5% dextrose (glucose) solution and slowly infused over 40-60 minutes. You can also dilute the required dose of the drug, just dissolved in the solvent, in 20 ml of 0.9% sodium chloride solution and slowly inject intravenously.

For subcutaneous and intramuscular administration, it is not necessary to additionally dilute the necessary dose of the drug dissolved in the solvent.

For subcutaneous and intramuscular administration, the required dose dissolved in the solvent does not need to be further diluted.

CONTRAINDICATIONS

Hypersensitivity to the active ingredient or any excipient of the drug.

Do not use in animals with signs of renal failure.

Do not use in pregnant cats.

SIDE EFFECTS

There were no side effects and complications when using the drug GEPAX.

Symptoms of overdose were not registered.

In case of individual hypersensitivity of the animal to the components of the drug and the appearance of adverse reactions, it should be discontinued and symptomatic therapy should be carried out.

PACKAGING AND PRESENTATION

5 glass vials with a lyophilisate, of 95, 190, 380, 570, or 760 mg (corresponding to 50, 100, 200, 300, and 400 mg of ademethionine cation, respectively) and 5 glass vials with a solvent of 1, 2, 4, 6 or 8 ml, respectively, in polymer blisters or cardboard boxes.

SHELF LIFE

24 months.

STORAGE CONDITIONS

Store in dry dark place, out of the reach of children at +5° to +25°С.

The solution of the drug cannot be stored: after dilution, the drug should be used immediately.
